Differentiation of false- from true-positive ECG responses to exercise stress by thallium 201 perfusion imaging.

چکیده
false-positive and true-positive ECG responses to exercise stress. Thirty-five patients referred for coronary angiography because of an exercise test interpreted as positive for ischemic ST segment changes (1.0 mm or more reversible horizontal or downsloping ST depression), but who were asymptomatic or had a history of atypical chest pain, underwent repeated exercise testing in conjunction with 201Tl perfusion imaging. All patients received 1.5 mCi of 20’Tl at maximal exercise, and imaging began five minutes after exercise.

Thallium-201 Myocardial Perfusion Imaging
The sensitivity of myocardial perfusion imaging (MPI) using thallium-201 injected both at rest and during peak exercise was compared to simultaneously recorded 12 lead electrocardiography (ECG) for the detection of transient ischemia in 20 normal subjects and 63 patients with coronary artery disease (CAD). No significant perfusion defects or ECG changes were seen on either the rest or exercise ...
متن کاملSerial Myocardial Imaging after a Single Dose of Thallium-201
Although thallium-201 exercise scintigraphy has been established for the detection of myocardial ischemia and viability, little is known regarding the myocardial thallium-201 kinetics during angioplasty. Herein, we report a 77-year old man with angina pectoris, in whom serial myocardial imaging after a single dose of thallium-201 was helpful in identifying not only the culprit lesion and myocar...
متن کاملThallium-201 myocardial SPECT in a patient with mirror-image dextrocardia and left bundle branch block.
A 53-year-old male patient with a previous diagnosis of situs inversus with mirror-image dextrocardia underwent thallium-201 (Tl-201) stress-redistribution myocardial perfusion single photon emission computed tomography (SPECT). Electrocardiogram (ECG) obtained on right hemithorax revealed constant complete left bundle branch block. Tl-201 stress-redistribution SPECT images revealed abnormal pe...
متن کاملComparative sensitivity of the exercise electrocardiogram, thallium imaging and stress radionuclide angiography to detect the presence and severity of coronary heart disease.
The relative sensitivity and specificity of individual and combined noninvasive tests to detect coronary heart disease were evaluated in 75 patients with chest pain admitted for cardiac catheterization and coronary arteriography. Of the 75 patients, 56 had coronary heart disease. Exercise-induced ST-segment abnormalities (>1 mm) were found in 58%. In contrast, computer-processed exercise thalli...
متن کاملDiagnosis of coronary artery disease by exercise thallium-201 tomography in patients with a right ventricular pacemaker.
OBJECTIVES We sought to study the accuracy of exercise perfusion scintigraphy in patients with an implanted apical right ventricular pacemaker. BACKGROUND The specificity of exercise perfusion scintigraphy is decreased in patients with a left bundle branch block. Patients with a permanent ventricular pacemaker have a similar conduction abnormality that may also potentially result in similar f...
